This is the form the selectors work from. Fill it in once and you are on the list for every CAUP trial in your age group — you will not be asked to do it again next season.

Before you start

You do not need to upload anything here. Bring your original date-of-birth and identity documents to the trial itself, where they are checked in person.

Get your date of birth right. It decides your age group, and a wrong date found later can cost a player a whole season.
